Located on a 340-acre campus in Oak Forest, in the southwest corner of Cook County, the 70 bed-hospital meets the needs of patients who require acute care and comprehensive rehabilitation services.
Oak Forest Hospital is closely affiliated with three suburban clinics operated by the Ambulatory & Community Health Network and is home to Oak Forest Ambulatory Center specialty clinics, including podiatry, eye clinic and wound care. The rehabilitation unit provides comprehensive inpatient and outpatient people with disabilities and is accredited by the Commission on the Accreditation of Rehabilitation Facilities. Follow-up outpatient therapies and services are provided to assist patients in living full active lives.
In September 2007, Cook County, in partnership with Caremark and the National Association of Counties (NACo), introduced the new Cook County Prescription Drug Discount Card Program.
